COVID-19 Vaccination Proof and Exemptions Process - Due by Aug 9, 2021
For all students - the deadline to submit a copy of your vaccination card showing that you are fully vaccinated is August 9, 2021. All documents can be submitted below.
Bennett College allows for medical and religious exemptions only that has been notarized. No philosophical exemptions will be granted for mandatory vaccinations (COVID-19, MMR, DTP, TDAP, Meningitis. All students granted Medical or religious exemptions must submit proof of negative Covid-19 PCR test obtained within 5 days of move-in and will undergo regular Covid-19 surveillance testing.
Medical exemptions require a notarized letter or statement on letterhead from your health care provider (MD, DO, NP, PA) indicating the clinically supported reason you should not receive the required vaccinations. We will not accept medical exemptions from chiropractors or naturopaths.
Religious exemptions require a detailed written statement of personal beliefs AND a notarized statement on letterhead from your religious leader/clergy person. This information can also be found in the attached Student Handbook 2021-2022, page 19.
Students who have been confirmed for housing and have not submitted the required documentation by August 9, 2021are at risk of being dropped from on-campus housing. Commuter students will not be permitted on campus or attend face-face classes until proof of being fully vaccinated or receiving a validated medical or religious exemption by the Health Center.

We look forward to providing you exemplary service at the Bennett College Student Health Center. The Student Health Center provides a variety of medical and other health services to our students.
Prior to entering the College, each student is required to submit a report of her medical history, which must include a physical examination by a licensed physician and proof of required immunizations in accordance with the North Carolina Immunization Law, G.S. Article 6, Part 2. The Center must have complete immunization records for each student prior to arriving at college including three Diphtheria Pertussis Tetanus (DTP Vaccines), one Tdap (Tetanus Booster)received within the last 10 years, three hepatitis B immunizations, two MMR (Measles, Mumps, Rubella) vaccines, Meningococcal (Meningitis Vaccine) and three Polio vaccinations, for students born after 1994. Effective Fall 2021, Bennett College requires that students must have the COVID-19 vaccination. Each student is required to submit a report of her medical history signed by a licensed physician.

Additionally, the Bennett College Student Health Center requests the following medical forms which provide information that will help expedite health services, should the need arise. They include the Demographic, Emergency Contact and Insurance Form, Consent Disclosure Information, and a Verification and Authorization to Treat Form. Each form may be found in the attached packet.
