What You’ll Need:
✅ Original Birth Certificate
✅ License/Government ID
✅ Bennett Faculty/Staff/Student ID or Picture ID
✅ Social Security Card or Bank Card
📑 Bring 2 printed photocopies of all 4 documents
💳 Payment Details:
✍🏾 2 Checks Required:
💵 $45 – Payable to Guilford Register of Deeds
💵 $130 – Payable to U.S. Department of State
📄 Complete your DS-11 form in advance and print it for your appointment.
